Partnership Referral Form

This form should only be used by Local Authority Agencies to refer premises with fire safety concerns. The Agency should download and complete the form before emailing to the appropriate email address under Part 4 of the form.

For urgent referrals contact telephone numbers can be found under Part 4, however, this form must always be completed.
